This form must be submitted to The Graduate School three weeks prior to the defense date or by current semester deadline for graduation.

For faculty and staff use only. Doctoral Student's committee signs the form when the student's dissertation has been approved by the committee. Submit completed form by email as a PDF to gradsvcs@lsu.edu.
