This form is for a student to petition scholastic reinstatement.

Online portal where feedback in the form of the survey where students who have earned doctorate degrees from LSU can submit feedback about their experience over their LSU academic career.

Form to be filled out by graduate faculty for defense results.

This form must be submitted to The Graduate School three weeks prior to the defense date or by current semester deadline for graduation.

Students must have earned 9 hours of graduate residence credit in a degree program at LSU and be in academic good standing before submitting this form. Please attach form to this request.

Early in each semester or summer term, there is a deadline for submitting the “Master’s Application for Degree” to the Graduate School. Master’s candidates are required to submit the “Application for Degree” forms by the semester’s calendar deadline. Please attach form to this request.

For faculty and staff use only. Doctoral Student's committee signs the form when the student's dissertation has been approved by the committee. Submit completed form by email as a PDF to gradsvcs@lsu.edu.

This form is for Affiliate Graduate Faculty Nomination by Faculty.

Form filled out by faculty to academic renewal.

This form must be submitted to The Graduate School no later than the last day to add courses for the semester in which graduate credit is requested and student’s last semester of enrollment as an undergraduate. Please attach form to this request.

If circumstances prevent graduation, a “Master’s Application for Degree UPDATE” form must be submitted to the Graduate School by the designated application for degree deadline for the next semester in which the student plans to graduate. Please attach form to this request.

This form is intended for students who are changing from a Master’s to a PhD or vice versa.
