Prerequisites for Form Submission
Nominations must be made by two full members of the graduate faculty directly associated with the same graduate degree program as the nominee. Qualifications for appointment are listed on the Graduate School’s website. A current vita and letter of justification is required. Please do not submit vitas using the PS-36 format. The justification letter should specify how the nominee will directly benefit graduate education in the department or program.
