SEVIS Check-in Form Help Guide

As a new F-1 or J-1 student at LSU, one of the first things you need to do after arriving in the United States and/or Baton Rouge is to report your arrival to LSU and to the U.S. Department of Homeland Security. This step is completed via updates to your F-1 or J-1 record in the Student and Exchange Visitor Information System (SEVIS).

Advisors at International Student Services (ISS) facilitate your reporting in SEVIS; however, the advisors need information from you first. The SEVIS Check-in Form (accessible after login to your iTiger Portal) collects what is needed for ISS advisors to review before formally reporting your arrival in SEVIS. You must complete complete and submit the SEVIS Check-in Form by the deadline indicated on the form.

 

Materials and Information Needed

To complete your SEVIS Check-in Form, you will need the following:

  1. Electronic copies of your current status documents.
  2. Your local address in the United States.
  3. Your home country emergency contact information.
  4. Your U.S. emergency contact information (if you have one)


Current Status Documents

The following documents in jpeg, png or pdf format must be uploaded to your SEVIS Check-in Form:

  • Your current passport bio page (page with your picture and passport expiration info).
  • Your most recent F-1 or J-1 visa stamp page in your passport. 
  • Your Most Recent I-94 Admission Record or I-797 (the replacement I-94 you received from an Immigration Service Center if you applied for a change of status and received it). 
  • If you have dependents who arrived with you, you will need to provide a copy of their passport bio page.

Entry Visa - Your most recent F-1 or J-1 visa stamp page in your passport. (Canadians are visa exempt.) Please only upload this one page with the visa, as shown in the example below.

Proof of your F-1/J-1 status - Your proof of status is one of the following:
 
  • Electronic I-94 Admission Record for those who have entered the United States - see image below. Do not upload the "Travel History."
  • I-797 approval notice for those who applied for a change of status within the United States via an Immigration Service Center and received approval.
