ISS New Student Information Form Help Guide

As a new F-1 or J-1 student at LSU, one of the first things you need to do after arriving in the United States and/or Baton Rouge is to report your arrival to LSU and to the U.S. Department of Homeland Security. This step is completed via updates to your F-1 or J-1 record in the Student and Exchange Visitor Information System (SEVIS).

Advisors at International Student Services (ISS) facilitate your arrival reporting in SEVIS; however, the advisors need information from you first. The New Student Information Form collects what is needed for ISS advisors to review before formally reporting your arrival in SEVIS. Please complete and submit this form upon your arrival to the United States.

 

Materials and Information You Will Need

You will need the following information and materials to complete your New Student Information Form.

  1. Electronic copies of your current status documents
  2. Your local address in the U.S.
  3. Your home country emergency contact information
  4. Your U.S. emergency contact information (if you have one)


Current Status Documents

You will need copies of the following documents available as a jpeg, png or pdf to upload into your New Student Information Form:

  • Your current passport bio page (page with your picture and passport expiration info).
  • Your most recent F-1 or J-1 visa stamp page in your passport. 
  • Your most recent I-94 admission record (accessible at i94.cbp.dhs.gov/) or I-797 (the replacement I-94 you received from an Immigration Service Center if you applied for a change of status and received it). 
  • Your LSU I-20 for F-1 status or your LSU DS-2019 for J-1 status that includes your signature. 
  • If you have dependents who arrived with you, you will also need to provide copies of their documents.
