J-1 Requests and Forms

Below is a list of the most frequently accessed J-1 student forms and services. Note: International Student Services is in the process of transitioning to online forms. During the transition, please use the designated links to submit your forms to ISS for processing.  

• Submit Your Form to ISS - this is a general submission form. Some requests/applications require that you complete specific requests. These specific requests include:

  • J-1 Student Work Permission - used to apply for or renew work permission from ISS. J-1 students must have this authorization in writing before beginning/renewing on or off-campus employment.

  • J-1 Program Sponsor Letter Request - used to request a letter that is needed to apply for a U.S. Social Security Number.

  • J-1 Academic Training form

  • Travel Signature Request

  • Program Extension Request - see attachments for additional items needed to submit with this request type.

  • Reduced Courseload (RCL) - used if you are considering part-time enrollment for any reason. You will need to request and be approved for RCL to have a less than full-time course load and maintain legal J-1 status. Please meet with an advisor before initiating the process to drop below full-time.

• Secure Document Submission - used when an ISS advisor has asked you to submit additional information or you have forgotten to include an item with your request.

• J-1 Incident Report - used to notify ISS of any event that has impacted the health, safety or wellness of an LSU sponsored J-1 Exchange Visitor.
