CPT Document Checklist
Employer Offer Letter Requirements
Employers must provide an offer letter or cooperative agreement that includes the following:
- Letter must be on company letterhead
- Date of the letter no more than four months before the CPT request date
- Student name
- Company name
- Physical address of the experiential learning/training opportunity location
- Number of hours per week
- Start and end date
- Position title, position description, and duties
- Company representative signature (signatures can be a digital signature or scanned signature, typed signatures will not be accepted)
All of the information in the letter is required information to authorize CPT in SEVIS.
For any students graduating at the end of the semester, the CPT must end on the graduation date for that semester; the end date on the offer letter must not be dated later than the graduation date. CPT applications for candidates for the current semester graduation cannot be processed if the end date on the offer letter is not the graduation date.
The student must work during the entire period of CPT authorization, as determined by the offer letter start and end dates.
If the standard offer letter issued by your employer does not have the required information listed above, we will require a separate letter with the necessary information.
