| Academic Training |
For J-1 students seeking employment authorization related to their course of study.
Must be requested before
- the DS-2019 expires
- completion of the academic program.

| Change of Academic Level/Program |
For J-1 students who are changing their major and/or degree level with the university and need an updated I-20/DS-2019 to reflect that change. Students must have their department complete the Change of Level Academic Verification Form (see Attachments) before submitting to ISS. |

Dependent - Add J-2
|
For J-1 students who want to add a dependent (spouse/child) to their SEVIS record. Students must provide a copy of dependent passport biographical page and proof of funding in amount of $5,000 per dependent. |

Dependent - Remove J-2
|
For J-1 students who want to remove a dependent (spouse/child) from their SEVIS record. |
| Financial Update |
For F-1 or J-1 students whose funding has changed an need an updated I-20 / DS-2019 to reflect those changes. Proof of the funding must be uploaded with the request. LSU funding must be official award/offer letter for current and/or future funding. (See Attachments for Affidavit of Support, if family or other sponsor). |
| Incident Report |
Used to notify ISS of any event that has impacted the health, safety, or wellness of an LSU-sponsored J-1 student. |
| Program Sponsor Letter Request |
For J-1 students to request a letter that is needed to apply for a U.S. Social Security number |

Reduced Courseload Request
|
For J-1 students who are considering part-time enrollment for any reason must have authorization before dropping below full-time. Students must meet with an ISS advisor before starting the process with the university to drop below full-time. |

For F-1 or J-1 students who are unable to locate their most-recent I-20 / DS-2019 and need a printed copy. |
| Work Permission |
Before beginning or renewing any on- or off-campus employment, J-1 students must have written authorization. |
